In order to process your application we will need the following documentation. This should be submitted to: benneking5@gmail.com. Your documents will be shared with the AWARE Board of Directors only. Remember, AWARE is run by volunteers, therefore, it may take several days before you receive a response. We appreciate your patience.
- Membership Application (submitted on line)
- A copy of your adoption application and home visit procedures (e-mail)
- A copy of your spay/neuter policies including those for puppies (e-mail)
- Your policy on taking back animals you have adopted out.
- Two references (if possible) from humane societies/shelters/animal control with whom you work (one must be WI based)
- Two vet references (if possible) to attest to the vet work being done prior to adoption
